Initial Paperwork

Look for it in your email.

These forms, including others, will be directly emailed to you several hours before your appointment — after you have had a chance to speak with the therapist and schedule your initial consultation. It is important and required that you complete them before the appointment so the clinician can review them beforehand; otherwise the appointment will need to be rescheduled.

FAQ

Fees, cancellation & payment.

Key terms from the 2026 Counseling Fee Schedule and Payment Terms. For the full acknowledgment language, download the fee schedule PDF.

What are the 2026 counseling session fees?

Initial Assessment (60–75 minutes) is $350 per hour. Individual Therapy (50–60 minutes) is $300 per hour. Group Therapy is $150 per session. Other services — including report or letter writing, phone calls longer than 10–15 minutes, email, record copying, mailing, and sessions over 60 minutes not billed to insurance — are $300 per hour, prorated for time spent.

What is the cancellation and missed-appointment policy?

Late reschedule or cancellation of a weekday appointment with less than 24 hours' notice is $190. Late reschedule or cancellation of a weekend and/or in-office appointment with less than 48 hours' notice is $190. A missed appointment (not attended within 15 minutes of the scheduled time) is $190. Insurance does not pay for missed or late-cancelled appointments, and those fees do not count toward your deductible. Exceptions may apply in cases of emergency.

When is payment due, and who is responsible for the balance?

Payment is due at the time of service. You are responsible for the full fee until Enaya Counseling and Consulting LLC receives an explanation of benefits (EOB) from your insurance company, if applicable. Regardless of insurance status, you are ultimately responsible for the balance on your account for professional services rendered. A valid credit card or form of payment is kept on file; payment is collected through SimplePractice EHR or another approved vendor. Accounts not paid within 30 days of the service date may accrue interest at 2% per month (24% APR).

What if my session goes longer than 60 minutes?

A therapeutic hour billed to insurance is 45–60 minutes. If a session goes over 60 minutes — unless advised by the therapist or prescheduled as such — you (not the insurance) are responsible for the extended time at $5 per minute, or $25 for each 5-minute increment. Unplanned additional time is not submitted to insurance and is due the day of the visit. If insurance later covers the extended session, the amount charged will be refunded.

Will you complete FMLA, disability, or other paperwork?

FMLA, disability, letters of support, and similar paperwork are not completed unless you have met for at least 6–8 sessions, and only when the clinician believes the request can be supported based on what was presented at intake and in sessions. Paperwork and related administrative work are billed at $300 per hour. FMLA paperwork generally requires a minimum of 60 minutes. Short-term disability often takes longer and may require additional assessment. Administrative tasks such as record copies, faxes, releases of information, and non-clinical calls are also billed at $300 per hour, with a $300 minimum. Record requests may take up to 30 days.

How do Employee Assistance Program (EAP) sessions work?

If you are using an Employee Assistance Program, you may not be able to use more than 3 sessions — even if your EAP program lists more. EAP sessions are subject to therapist availability and discretion.

What if my card on file is declined?

If payment is denied or rejected by the credit, HSA, or debit card on file, it is your responsibility to update the payment method and pay before the next scheduled session. Failure to do so may result in termination of services or an extra fee of $50 per session. Checks should be payable to Enaya Counseling and Consulting LLC. Fee schedule rates and any good faith estimate are subject to change with advance notice.
